We have made the data used in our review freely available to the research community. You will need to email the research team detailing why you wish to obtain the data. Readers wishing to conduct possible analyses of this data will need to write a proposal for consideration by our steering committee [JBK, CM, TJC, JB, PBJ, RMM]. If accepted, this will also need to be approved by the Department of Health. All research conducted using this data set must acknowledge the steering committee in any publications eminating from the research.
